Job description

Position Summary: The Case Manager is responsible for providing comprehensive case management services, conducting educational lectures, and supporting clinical staff with various tasks. This role involves maintaining accurate case records, coordinating access to necessary services, and assisting with aftercare planning and referrals. The Case Manager also actively participates in clinical reviews, staff meetings, and professional development activities. Qualifications: Education/Experience: - Conducts didactics/educational lectures - Advises clients individually or in group on issues related to risk prevention, diagnostic screenings, and community resources that improve social determinants of health outcomes - R esponsible for establishing trusting relationships with patients and their families while providing general support and encouragement. - Assist clients in determining the steps needed to take in order to gain access to treatment services and support the client with identifying and adequately addressing barriers - Provide clinical services to screen, assess, and determine appropriate level of care - Assess social needs of program participants and provide resources to the ex...
