The Pharmacy Population Health Coordinator supports clinical pharmacy programs within the Beth Israel Lahey Health Performance Network (BILHPN) – Clinical Excellence Division. The role focuses on improving medication adherence, closing care gaps, and enabling performance in our value‑based payer contracts. Working under the direction of Clinical Pharmacy Specialists and pharmacy leadership, the coordinator carries out program operations, supports population‑health‑related pharmacy activities, and collaborates with interdisciplinary teams across the network. Job Description: Essential Duties & Responsibilities including but not limited to: - Review medication adherence, quality, and performance reports to identify patients needing intervention. - Conduct outreach to patients, providers, pharmacies, and care teams to address adherence gaps, medication access issues, and pharmacy benefit questions. - Assist with addressing barriers to medication access including coordination of refills and prescription assistance programs. - Route prioritized intervention opportunities to c...
