Job description

The Registered Nurse Clinical Educator specializes in diabetes care, provides expert guidance and education to patients, families, and interdisciplinary teams on effective diabetes management. The educator designs and delivers evidence-based training programs that promote self-care, lifestyle modification, and glycemic control. Through collaboration and clinical leadership, the RN diabetes educator supports improved patient outcomes and fosters a culture of continuous quality improvement and learning in diabetes care. The diabetes nurse educator serves to assess, assist, and educate diabetic patients in partnership with the provider. This includes: -Instruct patients on integrating diabetic medications with lifestyle, in order to achieve optimal diabetic control. -Educate patients on complications of diabetes. -Assist with psycho-social needs associated with diabetic management. -Assist patients with utilization of latest technologies in the ongoing treatment of diabetes. -Participate in individual and group education courses. -Acts as a mentor and resource to staff.
