Overview Helio Health's In Community Program (formerly Circare) is designed to help individuals and families, who have encountered certain obstacles develop the resources and supports to live a satisfying, naturally independent life. The Health Home Plus Care Coordinator works with individuals recovering from serious mental health disorders who are returning to the community. Pay Rate: $24.04 per hour: In addition to Helio Health’s comprehensive benefits package, this position is eligible for a $1,500 Sign-On Incentive! Responsibilities: - Will assist clients in becoming effectively and actively engaged in managing their healthcare to improve individual health outcomes while reducing avoidable hospitalizations and emergency department utilizations. - Will have overall responsibility for the HARP process including assessment, planning and development of the Plan of Care, as well as monitoring ongoing HCBS Services. - Will provide leadership by facilitating and leading multidisciplinary teams of medical, mental health, chemical dependency treatment providers, courts/legal entities, social workers, nurses, representatives of the Local Government Unit (LGU) as directed by the county AO...
