Summary: The primary responsibilities of the clinical pharmacist include providing comprehensive pharmaceutical care to all patients within Baystate Health. Under the direction of the Pharmacy Manager, the clinical pharmacist will perform a variety of duties including; age appropriate clinical pharmacy services, evaluation of medication orders for appropriateness, monitoring and patient follow up; drug information consultations, and support of the pharmacy department operations. The clinical pharmacist will maitain competency, training, and ongoing education of pharmaceutical knowledge, integrated technology, and practice model changes. It is the expectation of the clinical pharmacist to complete and document the appropriate tasks required to maintain patient specific record keeping in an electronic database. Such documentation may include Clinical Interventions, Safety Reporting Events, etc. The clinical pharmacist is also expected to adhere to all departmental and organizational policies and procedures as well as adherence to all the applicable laws and regulations pertinent to the proper management of pharmacy practice.
