Reliance Hospice and Palliative Care is currently hiring for a Full-time Hospice RN Case Manager to support our patients around Orange County (Irvine, Lake Forest, Huntington Beach, Newport Beach, Fountain Valley) . Hourly salary starts at $53.00 and up, depending on years of experience. Current or Prior experience working at a Hospice Setting/Facility is highly preferred. The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. Job Responsibilities: - Completes an initial, comprehensive and ongoing assessments of patient and family to determine hospice needs. - Assesses and evaluates patients status. - Follow plan of care and participate in revising the plan of care as necessary. - Regularly re-evaluating patient and family/caregiver needs - Provides education, instructions and support to the patient and family on medication and patient care related needs. - Acts as Case Manager when assigned by Clinical Supervisor/Nu...
