Manage symptoms, administer medications, and provide emotional support to patients and their families. Collaborate with the interdisciplinary team to develop and implement individualized care plans. Key Responsibilities: Provide direct patient care, including assessment, medication administration, wound care, pain management, and symptom control. Develop and implement individualized care plans based on patient needs and preferences, in collaboration with the interdisciplinary team. Educate patients and families on disease processes, symptom management, and end-of-life care options. Document patient assessments, interventions, and responses accurately and thoroughly.
