Summary: - Provides support to direct care staff. - Nurse-led appointments for chronic disease management. - Procedures which may include IV’s, dressing changes, and others. - May include medication, infusion and immunization administration. - Facilitates patient flow. Advocate for patients as needed. - Patient education. - Organizes patient health information and updates electronic health records. - Maintains emergency equipment and other resources needed to provide safe, high quality patient care. - Provides management of pharmacy refill program for patients. - Other duties as assigned. Schedule: Fulltime position - Working 60 hours every two weeks | Onsite Primarily Mon-Fri, 12-hour shifts, 7:30AM to 8:00PM| Every 3rd weekend typically 8:45AM to 4:30PM Pay and Benefits: - Starting pay begins at $33.98 per hour; exact wag...
