Description Teller County's Public Health & Environment Department in Divide, CO is seeking an experienced nutritionist to deliver WIC services to participants by determining and documenting eligibility, issuing food benefits, maintaining participant and program electronic records and paper files related to participant certification, interview participants, provide nutrition education with behavior change goals, and develop individual care plans with a sound understanding of nutritional concepts. What You'll Do: (1) Coordinate and implement all WIC program operations for Teller County in compliance with local, state, and federal regulations. (2) Determine eligibility by conducting interviews, health screenings, and nutritional assessments, including height, weight, and hemoglobin measurements. (3) Act as Case Manager to enroll participants, analyze nutritional risk factors, and tailor food benefit packages to individual needs. (4) Provide nutrition education and counseling, collaborating with participants to set behavior change goals and monitor progress.
