Team Manager – Transition Team Permanent: Grade 6 - £54,495- £64,811: Consultation grade - subject to formal evaluation under the Pay Equity Review Working 36.5 Hours per week: We are seeking an experienced and motivated Team Manager to lead one of our Transition Teams, supporting young people with disabilities and additional needs as they move from Children’s Services into Adult Services. This is an exciting opportunity to join an evolving service. The Transition Team is currently operating on a city‑wide basis and will transition to a North and South‑based model, aligned with the future structure of Younger Adults provision. The postholder will play a key role in leading through change and shaping future ways of working. As Team Manager, you will provide strong leadership to a team of social workers and practitioners, ensuring high‑quality, strengths‑based transition planning in line with relevant legislation and statutory guidance.
