JOB DESCRIPTION: SUMMARY OF POSITION The Clinical Care Coordinator is responsible for managing a caseload of clients, documentation and charting in electronic health record, communicating with multidisciplinary teams, educating community on available programming, and ensuring adequate patient care, with a main focus on veterans, military and their families receiving quality outpatient counseling services. They also review recommendations from the patient's mental health care providers and coordinate treatment and community resources available to the patient through the Military Services program and partnered organizations. Military cultural competency and a well-developed understanding of community resources for outpatient counseling is crucial.
