Scheduled Hours 40 Position Summary: Coordinates the care of patients for a designated service throughout the continuum of care; serves as a clinical resource in area of specialty or expertise. Job Description: Primary Duties & Responsibilities: Provides direct patient care including assessment, planning and evaluation of patients. Completes health history on patients and conveys findings to physician. Documents all patient interactions appropriately within EHR system. Provides diabetes education to patients, families, and caregivers. Analyzes blood sugar trends and recommends dose/technology changes. Develops and coordinates diabetes education material and programs. Provides phone consultation for prescription refills, sick calls, education and other patient concerns. Attends outpatient clinic sessions with providers to provide care, education and comprehensive assessment of patients. Coordinates patient care, orders diagnostic tests, convey findings to physician, assists in coordinating patient appointments and assists with referral to outside sources.
