Community Treatment Order Coordinator (TFT 1.0 FTE)

Job description

The Community Treatment Order Coordinator will provide consultation to community, schedule 1 and specialized hospitals. They will coordinate services with treatment providers for patients who are considered appropriate for or may be under Community Treatment Orders. In order to accomplish this task, the coordinator will develop the necessary linkages within the Mental Health system and other community agencies and systems in order to provide education, support, advice and leadership with respect to the use of Community Treatment Order in the Mental Health system. DUTIES: QUALIFICATIONS: - Lead and organize in coordination/cooperation with physicians, and treatment teams, a comprehensive individualized plan of care for patients identified as appropriate for Community Treatment Orders including case conferences, liaise with treatment providers and resources community case managers and support to patients and families. - In coordination/cooperation with physicians and treatment teams, assist in developing a comprehensive individualized plan of care for patients identified as appropriate for Community Treatment Orders.
