Description Are you looking to be a part of a company that makes a positive difference for others? Arms Acres has several opportunities to do so! Arms Acres is a licensed facility providing inpatient and outpatient treatment for those suffering from addiction to alcohol or drugs. Sign-on bonus eligible: FUNCTION: To provide comprehensive clinical case management services that include: assessments; individual, group and family treatment; treatment plan coordination; referent relations; discharge planning; and intervention with client support systems, including social service programs and managed care. Provide service in House and in the Community. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties as required. Provides/ensures comprehensive evaluation of patient's functioning as defined by facility policy and procedures. Coordinates the development and implementation of the multidisciplinary treatment plan; coordinates treatment plan review; performs these activities within established time frames.
