Overview Pay Range : $31.25 - $33.17 hourly Location: 1934 Old Gallows Rd Ste 210, Vienna, VA 22182 Schedule: Monday to Friday: Embark Behavioral Health in Vienna, Virginia is seeking a Case Manager to support our Outpatient (OP), Intensive Outpatient (IOP), and Partial Hospitalization Program (PHP) services. As a Case Manager, you’ll lead the coordination of education success plans, conduct intake assessments, facilitate PHP sessions, and manage ongoing communication with families, schools, and the clinical team. You’ll also support medication management, track attendance and treatment progress, and contribute to thoughtful discharge planning. Responsibilities: Conduct education assessments upon intake and develop individualized Education Success Plans tailored to each student’s needs. Coordinate and facilitate weekly or as-needed check-ins with students, while maintaining communication with school contacts to support academic accommodations and track attendance. Attend staff meetings, and assisting with student conflict resolution and case management as needed.
