Primary Care Advanced Practice Provider (PA-C/ARNP) – Compass Clinic Position Overview: We are seeking a Primary Care Advanced Practice Provider (PA-C/ARNP) who thrives in a value-based, patient-centered care environment . The ideal candidate will work closely with a dedicated interdisciplinary team— including a Physician, nurse care manager, and clinical pharmacist—to deliver high-touch, coordinated care to a defined patient panel. This is not a typical fee-for-service role. Our providers focus on long-term outcomes rather than visit volume. In this role, you will collaborate with other physicians, nurses, therapists, and pharmacists to deliver comprehensive care to patients enrolled in value-based care programs, such as Medicare Shared Savings Programs (MSSP), and MedAdvantage. Your contributions will be pivotal in reducing the total cost of care, improving quality metrics, and ensuring equitable access.
