Senior Associate, Growth & Business Development

Heritage Health Network (HHN) is looking for a Senior Associate, Growth & Business Development to drive expansion of our Enhanced Care Management (ECM): program through strategic healthcare partnerships. This is a: high-impact, field-based role where you’ll work directly with PCPs, IPAs, hospitals, ED teams, and behavioral health providers to generate referrals, build long-term partnerships, and grow membership across our health plan network. What You’ll Do: Build and manage relationships with: clinical and community partners to drive referral pipelines Conduct regular: in-person outreach and represent HHN in the field Identify and close new partnership opportunities, including executing MOUs: Educate partners on how HHN improves P4P performance, HEDIS measures, and quality outcomes Own and manage a: CRM pipeline: , tracking referrals through enrollment Partner cross-functionally with Operations and Care Management to ensure strong partner experience What Success Looks Like: Consistent: monthly ECM enrollment growth Strong, active: referral pipeline and partner engagement New: partnerships launched and producing referrals Measurable impact on partner: quality and performance metric...
