Job Purpose: To provide compassionate, dignified, and person-centred care to clients in their own homes, supporting them to live independently and safely within their community for as long as possible. The Healthcare Assistant will deliver care in accordance with individual Care Plans and organisational policies, reporting any changes in a client’s condition or wellbeing to the Care Manager or designated supervisor.
