Duties and Responsibilities: - Assessing clients needs through the intake and preliminary screening process. - Connecting clients and/or families with services and resources. - Ensuring continuity of care among the client’s treatment team. - Facilitating internal and external collaboration to develop person-centered and family-centered integrated treatment planning. - Assessing and assisting clients who have identified Social Determinants of Health (SDOH). - Work closely with internal and external referral resources. - Rotation of intake weeks for new inquiries and referrals. - Assisting clients with updating intake paperwork and ROI on an annually bases. - Documenting and maintain accurate and timely c...
