Description Transitional Care Coordinator Position Summary: Supports the overall business plan and strategic direction of the organization by facilitating intake processes for durable medical equipment and infusion pharmacy patients and providing internal sales support that augments the effectiveness of partnership development efforts in the field and that drives high-quality patient care and facilitates the efficient and accurate processing of patient accounts.
