Atlantic Health System is seeking a Per Diem Social Worker for Case Management at Chilton Medical Center. Essential Function/Responsibilities: - Provide Social Work coverage to facilitate discharges, meet with patients and families to assess and provide discharge planning support, resources, referrals, transfers to Extended Care and inpatient psychiatric facilities. - Work closely with the Healthcare Team to implement the plan of care, obtain insurance authorizations as required, communicate with community resources and area facilities. - Phone, Fax and Electronic documentation a must.
