Primary Care - Extended Hours APP

Job description

Overview This Advanced Practice Clinician provides acute care to clinic patients on an extended hours basis ( regularly to include early mornings, evenings and weekend hours ). The APP will work with a team of dedicated providers to ensure such patients receive the care and support that they need. The job is a non-paneled position working at Legacy Medical Group: Camas located at 1625 SE 192nd Ave Ste 100, Camas, WA. This position supports Legacy Medical Group Primary Care providers and their respective patient panels. This is a fully in-person position available to qualified clinicians licensed in the states of WA and OR. Responsibilities: GENERAL ACCOUNTABILITIES and ESSENTIAL FUNCTIONS: - Provides in-person acute care (virtual, when appropriate), emphasizing accessibility, health promotion, illness prevention and continuity of care to patients presenting with both minor and complex health issues. - Regular hours to include early morning, evening and weekend hours to accommodate patient needs. - Performs minor procedures as appropriate in an acute care setting. - Appropriately triages patients to the proper site of care and works with support staff to schedule patient for appropr...
