Social Worker- Acute | The Pavilion at Williamsburg Place | Williamsburg, Virginia About the Job: The Social Worker I plans, directs, defines and coordinates individualized and group therapeutic support and services to patients with behavioral problems, psychiatric disorders, post-traumatic stress disorder, substance abuse and physical and emotional abuse. Conducts individual and group therapy, crisis intervention, case management, and education. Roles and Responsibilities: · Conducts social history assessment of the patient through observation and patient/family interview, obtaining information on the patient’s needs, skills, interests and limitations for the development and implementation of the treatment plan. · Coordinates and plans programs and activities to meet patient’s social and emotional needs. · Promotes recovery, symptom reduction and the achievement of the highest level of social functioning. · Provide crisis intervention and assist families in understanding the implications and complexities of a life situation and its impact on lifestyle. · Participates in multidisciplinary treatment team meeting to discuss patient assessment, treatment plan and ongoing discharge pla...
