Scheduled Hours 40 Position Summary: Supports the development, implementation, and ongoing maintenance of HIV testing sites across emergency departments and community-based clinical settings. This role assists with recruiting new testing sites, implementing standardized testing and data protocols, and training clinical and non-clinical staff. Helps ensure quality assurance processes are followed, supports accurate data collection and reporting, and facilitates timely linkage to care. Job Description: Primary Duties & Responsibilities: Conducts a comprehensive sensitive assessment with the client and family/support network as appropriate, in order to develop a plan of care to meet identified needs. Arranges and coordinates service referrals for the client/family as needed; provides ongoing monitoring of the activities implemented to carry out the plan of care. Refers clients/families to public benefits programs as eligible. Provides regular contact with enrolled clients and documents all contacts and service coordination activities in client records as required by program guidelines.
