Job Title: Family Reentry Support and Help (FRESH) Care Manager Location: Wadesboro, NC: Hours: 40 hours/week, Full Time, M-F, Exempt Coastal Horizons Center, Inc. is seeking a qualified Care Management professional for our Family Reentry Support and Help (FRESH) program. We have this specific opportunity located in: Anson County, NC (Wadesboro, NC) Primary duties include providing case management and related services focused on the child-parent relationship and the re-entry/transition process for incarcerated parents and their children, both within correctional settings and in the community. This is a hybrid position ; the FRESH Care Manager works at the prison, specifically Anson Women’s Correctional Facility, and from home. Qualifications: Education : A bachelor’s degree in human services or a related field. A master’s degree in social work or a related field is preferred. Certifications : Preference is given to individuals with North Carolina Peer Support Specialist certification and a minimum of 2 years of experience, and/or individuals registered with the North Carolina Addiction Specialist Professional Practice Board (NCASPPB).
