T120 Team Leader: - Maximum Term till 30 June 2027 - Full-Time Opportunity - Based in Carnarvon ABOUT THE ROLE: The T120 Program helps young people make positive changes in their lives by working collaboratively with their families and local organisations. It provides targeted support to help participants strengthen their skills, build resilience, and improve their overall wellbeing. The Team Leader will work in partnership with the Target 120 Team (Carnarvon) guiding and leading the Family Support Worker and Youth Support Worker. The Program Leader will engage and support families in home or community to explore and understand family needs and aspirations through the development of the Integrated Family and Individual Support Plans. The Program Leader will take a lead role in establishing and maintaining key stakeholder relationships and delivery of program deliverables and/or outcomes as per Grant Agreement within nominated timeframes.
