Occupational Therapist (Per Diem) - PACE

Job description

This is an on call/per diem position that will be supporting all western WA PACE sites. The Occupational Therapist (OT) is responsible for OT evaluations and development and implementation of the OT plan of care in accordance with PACE policies. The OT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The OT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes. The OT assumes responsibility for participants treated by the OTA and OT Aide.
