Referral Coordinator

Job description

Under direct supervision, responsible for assisting with coordination of referrals, authorization, scheduling of patients and record keeping. EDUCATION: Associate's Degree degree in health related field is required. EXPERIENCE: Five (5) years experience in patient care setting is required. - Triage's patient inquiry and physician referral calls to coordinate intake of new patients; completes new patient pre-registration and scheduling. - Completes clinical assessment based on a phone interview with patient, family members or patient's physician; provides educational information. - Obtains patient records and full demographic data; enters the information into the computer for pre-registration. - Updates and relays information to patients or physicians about clinical activity. - Maintains the schedule for residents and faculty. - Assembles records for chart audit. - Performs all other duties as assigned. - Ability to operate a multi-line telephone system while providing excellent service. - Strong organizational and attention to detail skills. - Ability to establish and maintain effective working relationship with patients, employees and the public. - Ability to maintain strictest co...
