Position: Social Worker Position Summary: The Social Worker in the Case Management department at Lumeris plays a vital role in addressing the psychosocial and socioeconomic needs of members across the continuum of care. This position partners with an interdisciplinary care team, members, and their caregivers to coordinate services, connect individuals to community resources and plan benefits, and support effective case and disease management, including discharge planning. Through assessment, advocacy, and intervention, the Social Worker helps members navigate the complexities of their health and social situations, empowering them to make informed decisions and achieve improved health outcomes.
