Company: OhioHealth at Home in partnership with Compassus Position Summary: The Patient Care Coordinator's primary function is to schedule patient services and coordinate with staff. Position Specific Responsibilities: - Schedules and appropriately documents patient schedules in a timely manner. - Participates in coordinating care with management and patient interdisciplinary team. - Provides effective communication to patients, staff members, other health care professionals, and referral sources. - Responds to agency and patient needs in a professional and creative manner. - Performs other duties as assigned. Education and/or Experience: - High school diploma or GED required. - Experience with patient scheduling highly preferred. - Prior customer service preferred. Skills: Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities.
