We are seeking a Licensed Practical Nurse to join our team at our program in Philadelphia, PA. Shift: Monday-Friday 8:30am-4:30pm: As a Licensed Practical Nurse (Group Home Visiting Nurse) at AVS Merakey, you'll provide direct nursing care to individuals with developmental and physical disabilities. Your responsibilities will include traveling to our group homes, clinical assessments, monitoring acuity, teaching staff on residents care plan and medical needs, delivering medications to group homes, updating MAR and Treatment logs. Essential Job Functions: - Visit group homes, deliver medications, update MARs and Treatment Logs, conduct nursing assessments on residents to identify for signs and symptoms of illness, and report findings to the nursing supervisor. - Identify health issues and follow through to resolution, while documenting all actions. - Review, transcribe, and administer physician's orders accurately. - Collaborate with various healthcare professionals and specialists to coordinate care, while keeping the nursi...
