The Social Worker will coordinate with all parties involved in the care of the patient to provide effective and culturally competent care and treatment for people living with HIV and other chronic diseases. The Social Worker will provide care in accordance with the legal scope of practice and within established standards of care, policies and procedures. ESSENTIAL DUTIES AND RESPONSIBILITIES: Case Management: Perform patient care functions within the scope of competency as requested by health care provider(s) following established protocols, policies and procedures.
