Department: 03110 AHNMG Pediatric Endocrinology Ctr: 1062 Forsyth St - Pediatrics: Endocrinology Status: Full time Benefits Eligible: Yes Hou rs Per Week: 40 Schedule Details/Additional Information: Mon-Fri Pay Range: $35.50 - $53.25 Major Responsibilities: A. Responsible for providing diabetes self-management training and education to patients and their significant others. 3)Implement and document teaching plan, educational interventions, and follow-up plan based on the individualized assessment for patients and their families and / or significant others per current program educational curriculum. 4)Collaborate with patients and their families and / or significant others in establishing mutual learning goals and behavior change goals. 5)Document evaluation of outcomes and revises interventions and follow-up plan for patients and their families and / or significant others. 6)Assure education is based on current program curriculum and input from physicians. 7)Communicate education and follow-up recommendations to physicians and other health care providers within the multidisciplinary team. 8)Bill third party payers for diabetes outpatient self-management and training, utilizing appr...
