Wayne, ensuring a smooth, professional, and compassionate admission experience from start to finish. What You’ll Do as a Home Health Admissions RN: - Conduct comprehensive initial assessments and evaluations to determine patient eligibility and care needs. - Initiate the Plan of Care in accordance with physician orders, clinical standards, and regulatory guidelines. - Document all patient assessments, clinical findings, and admission records accurately and promptly. - Serve as a liaison between referral sources, patients, families, and the interdisciplinary team to ensure a smooth and supportive onboarding process. - Maintain compliance with all applicable federal, state, and agency policies. -...
