THIS POSITION IS REMOTE/WORK FROM HOME WITH 50% TRAVEL IN THE FOLLOWING ILLINOIS COUNTIES: CHAMPAIGN, VERMILION (NEARBY CITIES INCLUDE: CHAMPAIGN AND DANVILLE). THIS ROLE WILL SUPPORT AGING AND PHYSICALLY DISABLED MEDICAID MEMBERS BY MAKING HOME VISITS 1 X 90 DAYS, AND BASED ON RISK STRATIFICATION POTENTIALLY CONTACT TELEPHONICALLY 1 X 30 DAYS. THE WORK SCHEDULE IS MONDAY - FRIDAY 8AM - 4:30PM. Position Purpose: Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans/service plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs.
