Job description

Overview Promotes optimal patient outcomes by assisting with medication reconciliation. Interviews patients and reviews medication-related documentation for the purpose of compiling a medication history for patients being admitted to the hospital. Compiled history information is entered into the patient’s electronic health record for the purpose of facilitating medication reconciliation by the admitting provider. May perform a variety of functions which impact the medication reconciliation process and patient care. Seeks assistance/clarification from a pharmacist with any questions or issues that may arise. Responsibilities: 1. Interviews patients or caregivers to collect information regarding all current home medications;identifies when a discrepancy related to inappropriate dosing, frequency, or schedule for a homemedication exists, and uses this knowledge to obtain clarification to ensure accuracy of the homemedication list and to facilitate the timely/accurate collection of data.2.
