Extended Care RN Department: Participant Care: Employment Type: Full Time: Location: Ypsilanti, Mi: Reporting To: Jennifer Calvert: Compensation: $69,000 - $72,600 / year Description: Schedule: Exempt, minimum 40 hours per week | 7 days on/7 days off on call rotation with weekend hours Department: Participant Care | Huron Valley PACE | Ypsilanti, MI What You'll Do In This Role: Take your nursing impact beyond the traditional schedule. As an Extended Care Nurse at Huron Valley PACE, you’ll provide essential after-hours and weekend care, ensuring participants receive timely, compassionate support when they need it most. Working a unique 7-on/7-off schedule, you’ll take nursing and intake/referral calls, assess patient needs, and coordinate both scheduled and as-needed visits—helping manage complex conditions and prevent unnecessary hospitalizations. In close collaboration with the interdisciplinary team, you’ll use your clinical expertise to assess, plan, and deliver care that enhances independence, promotes autonomy, and supports the overall well-being of the older adults we serve.
