GENERAL SUMMARY AND PURPOSE: Provides advocacy, education, assessments and interventions to patients and families in the inpatient hospital setting to achieve an optimal level of care. Coordinates with medical staff and outside organizations to establish specific treatment plans and facilitate transition planning to achieve quality clinical outcomes. SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE: Master Social Worker (LMSW) required as defined by their primary work state (Idaho or Oregon), Licensed Clinical Social Worker (LCSW) preferred. * All colleagues must provide licensure or proof of application in process for the secondary state within 6 months from hire date.
