Position Summary: St. Joseph’s Healthcare Hamilton’s Renal Transplant Program is one of six adult renal transplant centers in the province. At SJHH, we transplant 120-165 patients annually including living and deceased donor transplants. The Renal Transplant Clinic focuses on preparing patients with chronic kidney disease for their upcoming kidney transplant surgery. We also work closely with living kidney donors to prepare them for their kidney donation surgery. Once surgery has taken place, transplant patients return to the clinic for follow-up care, management, support, and education. The Renal Transplant Clinic is a dynamic multidisciplinary team consisting of Registered Nurses, Nurse Practitioners, Physicians, Psychologists, Pharmacists, Social Workers, Registered Diabetes Educators, Dietitians, and Administrative Professionals. The Primary Care Nurse is an integral part of the team responsible for the care management of patients donating or receiving kidney transplants.
