WHAT YOU WILL DO: The Support Coordinator is a non-exempt position responsible for referral and screening coordination and the delivery of case management services to at-risk families of child abuse and neglect to reduce the likelihood of placement disruptions as well as abuse or neglect incidents. Case management services include referrals to service providers, completing funding applications, in-home support, and other support services. - Coordinate referrals from community agencies and conduct screenings of cases. - Complete referrals to community service providers for children and caregivers, including therapeutic providers, tutoring services, and child development screenings. - Assist families with connections to available financial resources, including, but not limited to, relative/non-relative caregiver funding, level 1 licensing, food stamp applications, and social security applications. - Provide information, referrals, and assistance in loca...
