Job Details ChristianaCare is currently hiring a full-time Nurse Navigator II for the Outpatient Services at the Helen F. Graham Center located in Newark, DE. Work Schedule: - Monday-Friday, day shift - No weekends or holidays required PRIMARY FUNCTION: The Oncology Nurse Navigator provides individual assistance to oncology patient populations and their families and caregivers to help overcome healthcare system barriers. While using the nursing process, the oncology nurse navigator provides education and resources to facilitate and coordinate care of identified patients with cancer and collaborates with the team of health care professionals to ensure timely, high-quality care across the cancer continuum. PRINCIPAL DUTIES AND RESPONSIBILITIES: Coordination of care of identified patients with cancer. Monitors patients’ progress according to plan of care; consults with other health care professionals as needed. Provides feedback to health care team at regular intervals and as necessary.
