Our Company Abode Care Partners: Overview: In this full time, clinical physician lead role, under supervision of the CMO, you will have several clinical responsibilities, along with some specified administrative responsibilities. While providing day-to-day clinical care to residents residing in facilities as well as home bound patients, you will practice and role-model ethical and evidence-based decision-making to promote the vision and mission of the company to other practitioners and staff. Responsibilities: - Provides high-quality complex and geriatric care to residents and patients across settings in partnership with non-physician practitioner and other interprofessional team members to an identified panel of patients across various assisted living and skilled nursing facilities - Keeps high engagement with facility leadership teams, nurse practitioners/physician assistants, patients, and families - Provides collaborating physician duties for on average of 5 but up to 15 nurse practitioners and physician assistants (based on state regulatory parameters) to support them in their clinical roles - Based on state and regional regulatory frameworks, audits up to 10% of practitioner...
