Summary: Provides support to community youth and their families including informal counseling, care coordination, and overseeing the daily operations of the program's services. Responsibilities: Per grant requirements, completes surveys and/or assessments with clients and families and reports outcome data to program's Team Leader.Provides case management, care coordination, and informal counseling for clients and families enrolled in program.Develops and maintains daily activity schedules.Evaluates and assesses the needs of the clients and families.Transports clients and families to appointments and/or community resources, when needed.
