Overview SSM Health Rehabilitation Hospital - Neuro Transitional Center O'Fallon, MO: Physical Therapist: Per Diem: We are seeking a Physical Therapist at SSM Health Rehabilitation Hospital - Neuro Transitional Center, opening June of 2025. This is an exciting opportunity to change lives. The Neuro Transitional Center has 12 beds and is designed to provide intensive rehabilitation to individuals with an acquired brain injury or spinal cord injury who are unable to safely return home at the time of discharge from an inpatient rehab hospital due to the severity of their impairments. Currently, 93% of our patients return to their homes at discharge from NTC, and now will not have to leave the state to receive this level of service. Responsibilities: Position Summary: Evaluates all assigned patients and develops, implements and modifies the plan of care in accordance with the patient’s needs and collaboration with the transdisciplinary team. Responsible for directed transfer of skills learned by patients during formal therapy into the evenings, weekends and community through team collaboration, communication and training of the Neuro Rehab Coaches.
