Hospice Nurse Practitioner Lead - Part Time 48 hrs bi-weekly - weekdays 8am-4:30pm occasional on-call rotation. The Nurse Practitioner Lead position is responsible for providing compassionate, patient-centered care to individuals with life-limiting illnesses, emphasizing comfort, dignity, and quality of life. This position is responsible for conducting comprehensive assessments, managing individualized care plans, and prescribing treatments to control pain and other symptoms in collaboration with the hospice medical director and interdisciplinary team. This position is also responsible for serving as a clinical resource for patients, families, and team members, offering education and support while ensuring care meets regulatory, ethical, and professional standards.
