We currently provide skilled nursing, companion, therapies and personal cares for a wide variety of clients in Eastern Iowa. We proudly provide these services to veterans, and we are in need of a part-time Physical Therapist to expand the level of skilled care available to them. - 401(k) - Dental insurance - Flexible schedule - Health insurance - Paid time off - Training & development - Vision insurance Responsibilities of as a Physical Therapist include: - Provide high quality evaluation and treatment of neuromusculoskeletal disorders - Develop personalized care plans to improve mobility, strength, and overall function of patients in the home setting, involving patient in setting goals. - Communicate and collaborate with associated medical professionals to achieve patient goals - Clear, concise documentation in our Clear Care software system - Regular communication with physicians, nurses, case managers within Veterans Affairs as needed - Ability to work independently and autonomously, managing one’s own schedule - Supervise home health aides involved in patient c...
