JOB SUMARY: A Home Health Community Liaison Assistant drives patient census growth by building relationships with physicians, hospitals, and assisted living facilities to facilitate referrals DUTIES & RESPONSIBILITIES: Build and maintain relationships with referral sources such as discharge planners, doctors, and nursing home administrators. Promote home health, hospice, or home care services to potential clients and medical professionals. Attend community events, health fairs, and networking functions to increase agency visibility. Coordinate with hospital staff, families, and internal teams to facilitate smooth patient transitions from hospital to home. Maintain records of sales activity and track data related to patient referrals to ensure company growth goals are met.
