Cooling/Warming Center Case Manager

Job description

The Salvation Army Family Haven Shelter is looking for a Case Manager for the Warming Center. This individual will provide case management services to individuals and families involved in the Cooling/Warming Center program, develop case plans intended to increase clients functioning and resettle clients in permanent housing, and participate in all phases of case management advancing these plans. Ability to work with vulnerable populations and can navigate various resource systems. This is a full-time, 40 hours per week, $19.50 per hour.
