Important Work Model: This is a hybrid position that requires regular onsite work at Waukesha Memorial Hospital in Waukesha WI. This is not a fully remote role. Schedule : - Full time Monday through Friday - 8:00 AM to 4:30 PM - Weekend and holiday rotation The Patients You’ll Work With: This role requires you to identify trends recognize early signs of decline and intervene before escalation is needed. - Chronic conditions including hypertension heart failure and other complex diagnoses - Recently discharged patients needing close follow up and early intervention - Patients in programs focused on preventing readmissions and catching changes early - A mix of stable and higher acuity patients depending on program - Patient panels can be larger requiring strong organization and clinical judgment Why You’ll Love This Role: - Blend of technolog...
