Department: 11200 Carolinas Rehabilitation: Charlotte - Case Management Status: Full time Benefits Eligible: Yes Hou rs Per Week: 40 Schedule Details/Additional Information: M-F 8am-4:30 pm. Willingness to travel to all Carolinas Rehab locations. Weekend on call as needed Pay Range: $35.50 - $53.25 MAJOR RESPONSIBILITIES: - Provides social work services to individuals or groups including: screening, psychosocial assessment, discharge planning, determination of social work goals, appropriate concrete and/or therapeutic interventions and follow-up. Exercises a substantial degree of professional judgment, recognizing deviations from the usual patient/family functioning, anticipating problems and taking measures to maximize coping capabilities. Makes recommendations for safe discharge planning, incorporating psychosocial assessment in the context of acute and chronic health care conditions - Documents activity according to department and program protocols or standards and serves as an expert user of information technology platforms that support care management.
